Policy and Procedure
Evangel Academy maintains a
non-discriminatory policy based upon race, color and
national origin in the administration of it's admissions,
educational policies and procedures.
Admission Procedures
1. Submit an application accompanied by the registration
and testing fees.
2. Our office staff will call both parents and pupil for an
interview with the Administrator or office staff.
3. All forms must be completed and signed including
Parental Agreement Form, Student Agreement Form, Liability
Release Form, Medical History and Transportation Forms.
4. Parents will be notified of acceptance.
5. A medical Report must be completed by a Physician and
submitted to the school office. All students must have the
following immunizations: diphtheria, polio, measles and
rubella. In instances where a student needs medical
attention, the secretary will call the parent or family
doctor (in that order.)
